The Subject Of The Order Is The Provision Of A Service Called Collection, Transport And Management Of Municipal Waste From Owners Of Properties Where Residents Live And From The Selective Municipal Waste Collection Point - In The Jerzmanowice-Przeginia Commune From January 1, 2026 To December 31, 2026. The Subject Of The Order Is Detailed In The Description Of The Subject Of The Order (Opz) Constituting Annex No. 1 To Swz.3. This Procedure Is Covered By Option Law. The Ordering Party Pursuant To Art. 441 Pzp Provides For The Possibility Of Exercising The Option Right. 1) The Application Of The Option Law Means That The Amount Of Waste Forecast In The Opz May Increase By 20%, Increasing The Contractors Gross Remuneration Specified In The Agreement To A Maximum Of 20%. 2) The Rules And Conditions For Exercising The Option Right Are Specified In The Draft Agreement (Appendix No. 5 To The Swz). 3) The Ordering Party May Use The Option Because It Provided For The Option In The Contract Notice And In The Procurement Documents In The Form Of Understandable, Precise And Unambiguous Contractual Provisions. Description Of The Method Of Preparing Offers And Formal Requirements Regarding Submitted Statements And Documents: 1. The Contractor May Submit Only One Offer. 2.The Content Of The Offer Must Correspond To The Content Of The Swz. 3. The Offer Is Prepared In Polish On The Offer Form - In Accordance With Annex No. 2 To The Swz. Together With The Offer, The Contractor Is Obliged To Submit: 1) A Declaration In The Form Of The European Single Procurement Document (Espd), Referred To In Chapter Ix Section 1 Swz; 2) Commitment Of Another Entity And A Declaration In The Form Of The European Single Procurement Document (Espd) (If Applicable); 3) The Means Of Evidence In Question; 4) Declaration Of The Contractor/Contractor Jointly Applying For The Contract Regarding The Grounds For Exclusion Under Art. 5K Of Regulation 833/2014 And Art. 7 Section 1 Of The Act On Special Solutions For Counteracting Support For Aggression Against Ukraine And For The Protection Of National Security - Annex No. 9A To The Swz. 5) Declarations Of The Entity Providing Resources Regarding The Grounds For Exclusion Under Art. 5K Of Regulation 833/2014 And Art. 7 Section 1 Of The Act On Special Solutions For Counteracting Support For Aggression Against Ukraine And For The Protection Of National Security - Annex No. 9B To The Swz. 6) Proof Of Deposit Payment (In The Case Of A Deposit Submitted In The Form Of Sureties Or Guarantees); 7) Documents Confirming The Right To Sign The Offer, I.E. A Copy Or Information From The National Court Register, The Central Registration And Information On Business, Appropriate Powers Of Attorney (If Applicable). 4.
