The Subject Of The Order Is A Delivery For Employees Of The Health Care Complex In Sucha Beskidzka: Package No. 1 - Prepaid Cards: - 22 Pieces Worth Pln 300.00 - 28 Pieces Worth Pln 600.00 - 1 Piece Worth Pln 850.00 - 675 Pieces Worth Pln 1,000.00 - 2 Pieces Worth 1 Pln 050.00, The Total Nominal Value Of The Cards Is: Pln 701,350.00,1. Prepaid Cards Must Be Bearer Cards. 2. The Minimum Required Validity Period Of Cards And Vouchers Cannot Be Shorter Than The End Of December 31, 2026. , A Longer Period Offered Will Be Assessed 3. All Prepaid Cards Must Have The Same Expiry Date. 4. Prepaid Cards Must Be Able To Be Redeemed At All Retail, Service And Catering Outlets Throughout The Country That Have Card Readers. 5. Prepaid Cards Must Allow For Any Number Of Transactions Up To The Amount Of Funds Available On The Card. 6. The Ordering Party Does Not Allow The Possibility Of Charging Any Commissions On Transactions Concluded At Sales Points.7. Prepaid Cards Cannot Be Subject To A Limit On The Value Of Operations Performed Using Them. 8. Prepaid Cards Must Be Secured With A Pin Code. The Contractor Provides The User With The Opportunity To Change The Pin Code Free Of Charge.9. The Ordering Party Does Not Allow Charging Any Fees For The Use Of Prepaid Cards By Users (I.E. Commissions On Transactions Made Using An Electronic Card, Annual Fee For Card Use, Card Insurance). 10. The Contractor Provides The Opportunity To Check The Amount Of Available Funds In Real Time On The Prepaid Card And The Transaction History Free Of Charge, By Phone And Via The Internet. 11. The Contractor Provides Free, Automatic Activation Of Prepaid Cards. 12. The Contractor Provides The Possibility Of Blocking The Prepaid Card Directly By The User. 13. The Contractor Ensures The Possibility Of Issuing A Duplicate Card In The Event Of Damage, Loss Or Theft, The Cost Of The Duplicate Will Be Max. Pln 30.14. The Contractor Will Deliver Prepaid Cards In The Following Way:• The Cards Will Be Delivered To The Health Care Complex In Sucha Beskidzka, Ticket Office, Room 105.• Each Prepaid Card With An Assigned Number And Expiry Date Will Be Delivered In An Envelope Along With A Document Containing Information On How To Properly Use The Card, I.E. How To Block The Card, How To Check The Balance And Transaction History, Promotions And Discounts Granted By Individual Partners Commercial Card Users. The Envelope Will Be Marked With A Number And Face Value.15. The Ordering Party Stipulates That The Handling Of The Payment Card Loading Process Is The Responsibility Of The Contractor. 16. The Ordering Party Does Not Allow The Contractor To Charge A Margin Or Commission, Fees For Issuing Cards, Fees For Adding Funds To A Prepaid Card Or Fees For Account Activation.17. The Ordering Party Requires That The Possibility Of Withdrawing Cash From A Prepaid Card Be Blocked.18. The Ordering Party Requires That The Funds Transferred To Top Up Prepaid Cards Be Kept On A Dedicated Bank Account Covered B
