The Scope Of This Activity Includes: A) Purchase Of Equipment For The Endoscopy Laboratory At The Spzzoz In Żuromin In The Range, Quantities And In Accordance With The Requirements Of The Ordering Party Specified In The Description Of The Subject Of The Order Constituting Annex No. 12.1 B) The Offered Subject Of The Order Must Be Brand New, Free From Defects, Have Certificates Allowing For Use In The Territory Of The Republic Of Poland And For Use In Health Care Facilities In Accordance With Act Of April 7, 2022 On Medical Devices (Journal Of Laws Of 2024, Item 1620) - Declaration Of Conformity Marked With The Ce Mark For Medical Devices And Be Compliant With The Standards In Force In The European Unionc) The Contractor Is Obliged To Deliver Equipment To The Spzzoz In Żuromin, Install The Equipment And Train Staff In Its Use - If Applicable, And Both The Training And Delivery Are At His Own Expense Contractor, Transport Is Organized By The Contractor. The Risk Of Accidental Loss Or Damage To The Equipment/Equipment Is Transferred To The Ordering Party Upon Its Delivery To The Place Of Collection And Takeover By The Ordering Party. 2) Action 2 - Renovation And Retrofitting Of The System: Design, Build And Retrofit The Following Departments: Gynecology And Obstetrics, Together With The Maternity, Neonatal And Childrens Wards. The Scope Of This Activity Includes: A) Development Of The Concept Design And Design Documentation Based On The Functional And Utility Program Along With Reporting The Works/Obtaining A Building Permit, If Applicable, And All Other Administrative Decisions, Arrangements, Consents And Opinions Necessary To Implement The Subject Of The Contract And Annex No. 12 To The Swz - In The Part Regarding Measure 2, B) Carrying Out On Its Basis The Modernization And Retrofitting Of Hospital Wards In The Formula Design - Build - Equip, As Well As Ensuring Authors Supervision Over The Prepared Documentation, C) Delivery Of Equipment In Quantities And Assignment To The Branch In Accordance With The Functional And Utility Program, Meeting The Requirements Specified In Annex No. 12.2 To The Swz, A) Preparation Of Complete As-Built Documentation B) Performance Of All Necessary Activities At The Contractors Expense And Obtaining All Necessary Documents Necessary To Start Using The Facility (Including The Final Decision On The Use Permit - If Applicable).3) Action 3 - Replacement Of The Internal Elevator With An Elevator Adapted To The Needs Of The Hospital In Accordance With The Detailed Design.
