1. The Subject Of The Order Are Medical Transport Services By Ambulance With A Doctor For People With A Court Order For Compulsory Psychiatric Treatment.2. As Part Of The Order, The Contractor Will Be Obliged To Provide Transport By Ambulance With A Specialist Emergency Medical Team, Referred To In Art. 36 Section 1 Point 1 Of The Act Of September 8, 2006 On State Emergency Medical Services (Consolidated Text: Journal Of Laws Of 2025, Items 91, 637) For The Treatment Of A Given Person From His Or Her Place Of Residence/Stay To A Psychiatric Hospital Or A Hospital With A Psychiatric Ward And A Care And Treatment Facility In Accordance With The Decision Of The Competent Court And The Information Provided By The Ordering Party. The Contractor Should Have A Base For The Specialized Medical Rescue Team In The City Of Krakow.3. In The Case Of This Type Of Transport, It Is Often Necessary To Contact The Person Who Applied To The Court For Transport In Order To Confirm The Patients Whereabouts And, Depending On The Patients Condition, Cooperate With The Police, The Family Of The Person Referred For Treatment, Social Assistance, And The Fire Brigade In Order To Safely And Effectively Bring The Patient To The Ambulance.4. A Detailed Description Of The Subject Of The Contract And The Terms Of The Contract Are Included In The Draft Provisions Of The Public Procurement Contract, Which Will Be Included In The Content Of This Contract - The Contract Template And Constitute Annex No. 1C To The Swz (Hereinafter Referred To As The Contract Template). 5. The Ordering Party, In Accordance With The Instructions Resulting From Art. 95 Of The Act Specifies That It Does Not Require The Contractor Or Subcontractor To Employ Persons Performing Activities Related To The Execution Of The Contract On The Basis Of An Employment Relationship.
