Request For Proposalsthe Purpose Of This Request For Proposals (“Rfp”) Is To Select A Service Provider (Or Service Providers) To Perform The Services (“Services”) Described In Schedule A.Scope Of Servicesthe Proponent Will Provide Employee And Family Assistance Program (Efap) To Eligible City Employees, Volunteer Firefighters, Mayor And Council, Their Spouses, And Dependents.Counselling Servicesa) Provide Up To Five (5) Sessions Per Employee, Member Of City Council, Volunteer Firefighter, Spouse Or Dependent Family Members Per Year;B) Provide Services In The Categories Standard To The Employee And Family Assistance Program Field;C) Provide Services Within The Following Timelines:First Response To Queries Within 24 Hours Of Contact, Andfirst Appointment Within 48 Hours Of Receiving The First Call;D) Provide Services In The Language Of The Client’S Choice;E) Provide Services In A Neutral Location, In Any Municipality In The Lower Mainland And Fraser Valley;F) Clients Referred To Community Services Provided Throughout The Proponent Will Receive Preferential Placement On Any Wait List, And Will Receive Services At No Cost To The City. Any Cost For Community Services Will Be The Responsibility Of The Individual Client;G) Honour Special Requests Of Clients, Reasonably, By Providing Counsellors At Times Of The Day And Days Of The Week Required By Clients, Of The Gender Requested And Speaking The Language Requested By The Client; Andh) Case Management, Which Begins With Each Client Being Assigned To A Counsellor Who Carries Out The Professional Assessment And Becomes The Case Manager For The Duration Of The Contact With The Proponent. The Assessment Provides Both The Client And The Proponent With A Broader Picture Of The Client’S Issues And The Central Presenting Difficulty. A Treatment Plan Is Established With Goals Set For Short To Medium-Term Therapy. Should Referral For Long-Term Therapy Or Specialty Services Be Required, It Is The Case Manager Who Facilitates This And Who Then Carries Out A Series Of Prescheduled Contacts To Ensure A Quality Service Has Been Provided.Addendaif The City Determines That An Amendment Is Required To This Rfp, The City Representative Will Issue A Written Addendum By Posting It On The Bc Bid Website At Www.Bcbid.Gov.Bc.Ca And The City Website At Www.Surrey.Ca (Collectively, The “Websites”), And Upon Posting, Any Addenda Will Form Part Of This Rfp. It Is The Responsibility Of Proponents To Check The Websites For Addenda. The Only Way This Rfp May Be Added To, Or Amended In Any Way, Is By A Formal Written Addendum. No Other Communication, Whether Written Or Oral, From Any Person Will Affect Or Modify The Terms Of This Rfp Or May Be Relied Upon By Any Proponent.
