The Subject Of The Order Is To Carry Out A Specialized, Multidisciplinary Diagnosis Of Children Along With Specialist/Therapeutic Support For The Child After Diagnosis. The Order Is Carried Out As Part Of The Razem Superpocni! Project. Co-Financed By The European Union, Under The European Funds For The Silesian Voivodeship 2021-2027 (European Social Fund+), Priority: Fesl.07.00 - European Funds For Society, Measure Fesl.07.07 - Support For The Family, Children And Youth And Deinstitutionalization Of Foster Care. The Subject Of The Order Includes: A) Specialized, Multidisciplinary Diagnosis Of Children And B) Support Specialized/Therapeutic Services For Children After Diagnosisspecialized, Multi-Directional Diagnosis Of Children1) The Subject Of The Order Includes The Provision Of Specialized Diagnostic And Therapeutic Support For Children Enabling The Assessment Of Their Functioning And Improvement Of Their Motor Skills, Supporting And Stimulating Their Physical And Mental Development, As Well As Improving Their Social Competences Through Specialized, Multi-Directional Diagnosis Of Children (Type: Diagnosis Of Attachment Disorders And Trauma, Diagnosis Of Neurodevelopmental Disorders, Si Diagnosis, Speech Therapy Diagnosis, Tus Diagnosis, Adhd Diagnosis, Fasd Diagnosis).2) Diagnostic Support Will Be Provided For Each Child Individually By A Team Of Specialists Designated To Carry Out The Order. 3) Based On The Diagnosis, An Individual Therapeutic Support Package Will Be Developed For Each Child, Which May Include One Or Several Forms Of Therapy (Depending On The Identified Deficits And Difficulties Of The Child And Areas Requiring Support), Each In An Appropriate, Adjusted Number Of Hours. The Contractor Will Discuss The Results Of The Diagnosis With The Childs Guardian And Present An Individual Therapeutic Work Plan Together With An Opinion On The Identified Deficits In The Child In Written Form. 4) The Final Number Of Hours Of Therapy For A Given Child Is Approved Each Time By The Ordering Party.5) The Ordering Party Is Responsible For The Recruitment Of Children/Project Participants.6) Minimum Number Of Children Intended For Diagnosis: 60.7) Schedule For Providing Diagnostic Support To Children: A) Min. 15 Children Diagnosed In The Period From Signing The Contract To December 19, 2025 B) Min. 20 Children Diagnosed In 2026 C) Min. 25 Children Diagnosed In 2027.8) The Contractors Tasks Will In Particular Include: A) Providing Diagnostic Support For Children Reported By The Ordering Party Who Are Project Participants; B) Travel To The Place Of Support Implementation (As A Rule, The Childs Diagnosis Should Take Place At The Childs Place Of Residence Or, If The Nature Of The Diagnosis Does Not Allow Travel To The Childs Place Of Residence, In The Area Of ​​Bielsko-Biała); C) Place Residence Of The Children Recruited So Far: Bielsko County, Including: Czechowice Dziedzice, Kozy, Zabrzeg, Stara Wieś, Międzyrzecze Górne, Mazańcowice.
