1. The Subject Of The Contract Includes Services Tailored To The Specific Life Needs Of People, Resulting From The Type Of Their Illness Or Disability, Provided By People With Specialist Professional Preparation. Above Services Are Provided In Accordance With Art. 17 Para. 1 Point 11, Art. 50, Art. 50A Of The Act Of 12 March 2004 On Social Assistance And The Regulation Of The Minister Of Social Policy Of 22.09.2005 On Specialized Care Services. 2. The Contractors Tasks In The Field Of Specialist Care Services In Particular Include: 1) Teaching And Developing The Skills Necessary For Independent Life, In Particular: A) Shaping The Ability To Satisfy Basic Life Needs And The Skills Of Social Functioning, Motivating For Activity, Treatment And Rehabilitation, Conducting Trainings Of Skills Of Self-Service And Social Skills And Support, Also In The Form Of Assisting In Everyday Life Activities Such As: Self-Service, Especially The Performance Of Economic And Cleaning Activities, Including The Ability To Maintain And Run A Home,- Care For Hygiene And Appearance,- Maintaining Contact With Household Members, Peers, At The Place Of Study And Work And With The Local Community,- Organizing And Spending Free Time Together,- Using The Services Of Various Institutions, B) Interventions And Help In Family Life, Including:- Help In Dealing With Situations In Situations Crisis- Specialist Counseling, Crisis Interventions, Psychological Support, Therapeutic Conversations,- Facilitating Access To Education And Culture,- Consulting, Coordination Of The Activities Of Other Services For The Family, Whose Member Is A Person Obtaining Help In The Form Of Specialized Services,- Shaping Positive Relationships Of A Person Supported With Loved Ones,- Cooperation With The Family- Disabled, C) Help In Dealing With Official Matters, Including: - In Obtaining Social, Retirement And Disability Benefits, - In Completing Official Documents, D) Support And Assistance In Obtaining Employment, In Particular: - In Looking For Information About Work, Help In Finding Employment Or Alternative Occupation, In Particular Participation In Classes Of Workshop Workshops, Professional Activity Workshops, Professional Activity, Environment Self-Help, Centers And Clubs Of Social Integration, Work Clubs,- In Completing The Documents Needed For Employment,- In Preparation For A Conversation With The Employer, Supporting And Assisting In Contacts With The Employer,- In Solving Mental Problems Arising From Work Or Lack Of It, E) Help In Money Management, Including:- Learning To Plan Budget, Assisting Expenditure, Assistance In Obtaining Expenses, Fees, - Increasing The Ability To Manage Your Own Budget And Financial Independence. 2) Care - As Supporting The Treatment Process, Including: A) Assistance In Access To Health Services, B) Agreeing And Guarding The Dates Of Medical Visits, Diagnostic Tests, C) Assistance In Buying Or Ordering Medicines In The Pharmacy, D) Their Side Use, E) In Particularl
