Tenders Are Invited For Delivery Of Dental Materials For Fillings

1. The Subject Of The Order Is The Supply Of Dental Materials For Fillings, In The Scope Of Part 1 - Filling I2. The Employer Allows You To Submit Partial Offers (For Individual Parts Of The Order). Each Contractor Can Submit An Offer For One Part Or More Parts. One Contractor May Submit An Offer For All Parts Of The Order Specified In Annex 2 - In The Assortment And Price Form. It Is Not Allowed To Submit Offers That Do Not Include All Items Of A Given Part Of The Order3. The Offered Products Must Be Medical Devices Supplied As Factory New In Originally Enclosed, In A State Suitable For Use And Approved For Trading And Use In Poland In Accordance With The Act Of 07.04.2022 On Medical Devices And The Regulation Of The European Parliament And The Council (Eu) 2017/745 Of April 5, 2017 On Medical Devices, Changing Medical Devices, Changing The Management, 2001/83/Ec, Regulations (Ec) No 178/2002 And Regulation (Ec) No 1223/2009 And The Repeal Of The Council Directives 90/385/Eec And 93/42/Eec (Mdr) And Executive Acts, Correspond To The Requirements Of Law And Meet The Required Standards Enabling Them To Be Used For Intended Use. The Contractor Declares That The Subject Of The Contract Has All The Necessary Certificates, Approvals Or Approval Of The Relevant Offices And That It Is Free From Defects. 4. A Detailed Description Of The Subject Of The Order Together With Quantities Is Included In Annex 2 Of The Swz Assortment And Price Form. This Description Should Be Read Along With Any Changes To The Content Of The Specification, Which Is, For Example, The Result Of Answers To Contractors. The Quantities Given Are An Estimated Demand For The Duration Of The Contract And Are Used To Calculate The Price Of The Offer (I.E. Determining The Maximum Remuneration Of The Contractor). 5. In The Case Of Use In Swz And Its Attachments, Description Of The Subject Of The Order Of Trademarks, Patents Or Origin, Source Or A Special Process That Characterizes The Products Or Services Provided By A Specific Contractor, The Contracting Authority Allows Equivalent Solutions In Accordance With The Pzp Act. The Equivalent Product Are Products With The Same Composition, Dose As For The Reference Product. 6. The Contracting Authority Reserves The Right Not To Use The Amount Of Maximum Remuneration And, As A Consequence, Reduce The Maximum Value Of The Subject Of The Contract By 40 %. Thus, The Minimum Value Of The Benefit On The Part Of The Ordering Party In The Event Of Offering Goods In Accordance With The Requirements Of Swz And The Contract Is 60% Of The Maximum Value Of The Subject Of The Contract. The Contracting Authority Will Not Bear The Negative Financial Effects Caused By Such A Reduction, And The Contractors In This Respect Do Not Serve Any Claims Against The Employer. 7. The Contractor Will Provide The Subject Of The Contract With A Specialist Means Of Transport, In Accordance With Applicable Regulations, Standards, In A Way That Does Not Negatively Affect Usefu
