    Psychological Services Part No. 1-Covers About 70 Hours Of Individual Services, Place Of Provision Of The Service City Świdnica.Cpv 85300000-2 Social Work Services And Similar 85121270-6 Psychiatric Or Psychological Services 80340000-9 Special Education Services Special Education85312300-2 Services Of Supervision And Advice. Primary School As Part Of A Comprehensive Support Program For Families For Life. Services Will Be Organized Individually. The Duties Of The Contractor Of A Given Part Will Include: 1) Conducting Diagnosis Of Problems, 2) Conducting Therapy, Psychological Support, 3) Keeping Documentation, Including: Specialist Work Cards, Work Cards With The Client, 4) Keeping A Register Of Accepted Persons, 5) Performing Activities Being The Subject Of The Contract With Due Diligence, 6) Watching Over The Correct Implementation Of This Order, 7) Providing Services In Accordance With The Schedule, 8) On The Day Of The Month Of Statement From The Implemented Support And/Or Therapy, Taking Into Account: The Number Of Hours Allocated To The Service And The Number Of People Who Benefited From It, 9) In The Event Of A Need - Cooperation With Other Institutions, Entities. As Part Of The Service, The Service/Service Provider Is Obliged To Take Part In Meetings, Workshops, Or Additional Team Work Commissioned By The Ordering Party. The Draft Contract Is An Attachment To The Specification Of The Terms Of The Contract.
