Tenders Are Invited For Provision Of Physiotherapy Services For Residents Of The Paradyż Commune

1. The Subject Of The Contract Is A Service Consisting In Individual Physiotherapy For The Inhabitants Of The Paradyż Commune, Having A Referral From A Doctor For Treatments. 2. The Service Will Include Individual Therapy For Each Resident Of The Paradyż Commune 3. The Implementation Of The Tasks Will Be Conducted From Monday To Friday 7.5 Hours A Day. 4. Providing Physiotherapy Procedures Is To Be Performed By 2 People With: A. The Relevant Permissions (Qualifications) Of The Physiotherapist In Accordance With The Act Of September 25, 2015. About The Profession Of Physiotherapist; B. Entry In The National Register Of Physiotherapists; C. At Least 1 Person Annual Experience In The Profession Of Physiotherapist; 5. Employment Conditions: Self -Employment6. The Contractor Will Use: A) The Premises At. Ul. Opoczyńska 4 Made Available By The Ordering Party; B) Apparatus And Medical Equipment Of The Ordering Party; C) Pharmaceuticals And Disposable Equipment Of The Ordering Party; 7. The Use Of The Funds Specified In Point 6 May Only Take Place To The Extent Necessary To Provide Procedures Being The Subject Of The Contract. 8. The Contractor May Not Use The Funds Specified In Point 6 For The Purposes Of Granting Physiotherapeutic Fugitives. 9. Paid Services Are Not Allowed. 10. The Contractor Will Perform Treatments In The Field Of:- Electrotherapy;- Photocracy;- Magnetotherapy;- Ultrasound;- Laser Biostimulation;- Kinesitherapy,- Use Of Equipment: Ugul;- Massage. 11. The Contractor Must Have A Computer To Keep Medical Records And Keep A Patient Card And Confirm Physiotherapeutic Procedures In Writing And Electronic Form12. Caring For Order And Cleanliness At The Workplace; 13. Caring For Equipment And A Treatment Stand (Washing, Disinfection); 14. Referring To The Patient With Due Respect And In A Cultural Way; 15. Not Going Beyond Your Professional Skills When Performing Procedures, If The Scope Of Treatments Exceeds The Ability To Turn To More Competent Physiotherapists; 16. Guided By The Patients Good, Respecting His Subjectivity And Personal Dignity.
