Xi Item 1 - 3 Of Annex 1 To The Contract, Constituting Annex 3 To Swz; Package A - Protective Tests And Vaccinations Can Be Performed On The Unit (Municipal / Poviat Police Headquarters) Or At The Point Indicated By The Contractor, And Located In The City Where The Municipal / Poviat Police Headquarters Is Based. It Is Also Allowed To Indicate Research From The Package And The Point Located Outside A Given City, Where A Given Municipal/Poviat Police Station Is Based, Provided That This Is The City Of Katowice And The Contractor Will Show The Possibility Of Performing The Part Or All Of The Tests Indicated In This Package At This One Point. In Addition, The Contractor May Indicate As A Place To Perform The Research And Vaccination Points Located In Bielsko - Biała Or Częstochowa, Provided That The Contractor Demonstrates The Possibility Of Performing The Tests Indicated In This Package At This One Point. Package B Includes Other Tests Than Those Indicated In The Package And Ordered By A Doctor Authorized To Perform The Tasks Of The Occupational Medicine Service And Vaccinations Referred To In Point Xi Item 4 - 5 Of Annex 1 To The Contract, Constituting Annex 3 To Swz. Research Should Be Valued In Annex 3 To Swz - Price Form. The Contracting Authority Requires That The Same Services Appearing In Different Places In Annex 3 To Swz - The Price Form, Be Equally Valued. If The Contractor Offers Various Prices For Identical Services, Which Will Be Calculated By The Price Of The Offer According To The Formula Referred To In Section Xvii Point 2 Lit. C) The Specification Of The Terms Of The Contract, The Employer Will Recognize As A Different Mistake And Improve In Accordance With Art. 223 Para. 2 Point 3 Of The Public Procurement Law At The Lowest Price Of These Prices.
