The Subject Of The Order Consists Of 1 Part. 2. The Detailed Scope Of The Subject Of The Contract, The List Of Devices, Quantity And Method Of Execution Of The Order Include Attachments To Swz, Namely: 1) No. 1A To Swz - Price Form, 2) No. 1B To Swz - Description Of The Subject Of The Contract, 3) No. 3 To Swz - Draft Contract For A Successive Delivery. Annex 1A And 1B To Swz Are Attached To The Offer. The Contractor, By Completing The Indicated Items In Annex 1B To Swz- Opz, Confirms The Fulfillment Of The Requirements Specified By The Employer. 3. The Offered Objects Of The Order Must Be Complete, With A High Standard, Both In Terms Of Quality And Functionality, As Well As Free From Material, Construction And Legal Defects. 4. The Offered Subject Of The Contract Must Be Approved For Marketing And Use In Accordance With Applicable Law. 5. The Subject Of The Contract Must Comply With The Act Of April 7, 2022 On Medical Devices (I.E., Journal Of Laws 2012.974 Of 2022.05.09). 6. In All Swz Entries And Its Attachments, In Which Trademarks, Parameters, Sources Or A Special Process Are Indicated, Which Is Characterized By Products Supplied By A Specific Contractor Of Origin (Names Of Manufacturers Or Equipment), In Accordance With Art. 99 Para. 5 And When The Subject Of The Contract Was Described By Reference To Standards, Technical Assessments, Technical Specifications, Reference Systems, In Accordance With Art. 101 Para. 4 Of The Pzp Act, The Contracting Authority Allows Equivalent Solutions. In This Case, The Contractor Is Obliged To Prove In The Offer, In Particular By Means Of The Means Of Evidence, That The Proposed Solutions Equally Meet The Requirements Set Out In The Description Of The Subject Of The Contract. 7. The Purchaser Does Not Reserve The Possibility Of Applying For The Order Only By The Contractors Referred To In Art. 94 Of The Pzp Act. 8. The Purchaser Does Not Set The Requirements Referred To In Art. 96 Para. 2 Point 2 Of The Public Procurement Law. 9. Source Of Funding For Public Procurement: The Contract Is Financed From The Project Under The National Plan Of Reconstruction And Increased Immunity: Obtaining Accessibility To The Highest Quality Of Oncological Therapy And Diagnostics By Purchasing Equipment For Copernicus. Z O.O. In Gdańsk. Component D Efficiency, Availability And Quality Of The Healthcare System. Investment D1.1.1. The Amount Of Vat Is Not An Eligible Expenditure Under The Above -Mentioned Project And The Contracting Authority Will Finance It From Its Own Resources. 10. In Connection With The Financing Of The Purchase From The Project Referred To Above, It Is Required That The Purchased Assortment Should Be In Line With The Principle Of Non -Management Of Significant Damage To The Environment (Dnsh - Do No Significant Har).
