1. The Subject Of The Order Is The Delivery Of Brand New Aed Defibrillators In The Amount Of 25 Pcs. With External Abs Plastic Cabinets With Light Sound Alarm And Heating In The Amount Of 25 Items As Part Of The Task: Retrofitting The Ropczyce Commune In Equipment Necessary For The Protection Of Population And The Functioning Of Civil Defense. The Task Is Implemented Based On A Targeted Subsidy For The Implementation Of Own/Commissioned Tasks Under The Civil Protection And Civil Defense Program For 2025-2026. Equipment Should Meet The Requirements Of The Ce Zd Conformity Declaration. With Regulation 2017/745 On Medical Devices. The Contractor Provides The Supply Of Purchased Equipment To The Address Indicated By The Buyer, I.E. The Seat Of The Municipal Office In Ropczyce, Ul. Krisego 1, 39-100 Ropczyce. 2. The Contractor And The Delivery Of Products Are Required To Provide Documents In Polish Regarding Devices, I.E. Instructions For Use, Documents In A Paper Version That Allows You To Make Mandatory Entries Arising From Art. 63 Para. 3 Of The Act Of April 7, 2022 On Medical Devices (Consolidated Text: Journal Of Laws Of 2024, Item 1620), Warranty Card. 3. A Detailed Description Of The Subject Of The Contract, Including The Technical Description, Included In Annex 2 To Swz.4. The Contractor Is Obliged To Fill Out - Factory Specification Of New Defibrillators With External Cabinets - Point 2 Of The Offer Form Constituting Annex 1 To Swz. Selecting The No Or/Or Incomplete Completed Column Contractor Declaration May Cause The Contractors Call To Explain Or Reject The Contractors Offer. 5. If The Description Of The Subject Of The Contract Uses Markings Or Parameters Indicating A Specific Manufacturer, A Specific Product, Trademarks, Patents Or Origin Were Indicated, The Contracting Authority Allows The Use Of Equivalent Solutions, By Which Products Should Be Understood By Products With Parameters Not Worse Than Presented In The Description Of The Subject Of The Contract And Meet At Least In The Same Scope Of The Contracting Authority. 6. If The Description Of The Subject Of The Contract Refer To Standards, European Technical Assessments, Approval, Technical Specifications And Technical Reference Systems Referred To In Art. 101 Para. 1 Point 2 And Para. 3 Of The Pzp Act, The Contracting Authority Allows The Application Of Equivalent Solutions. 7. The Contractor Citing Equivalent Solutions Described By The Contracting Authority Is Obliged To Show In The Offer That The Products He Offers Meet The Requirements Of The Ordering Party. 8. Required Warranty Period For Defibrillators At Least 3 Years, On Cabinets At Least 2 Years From The Date Of The Final Reception Of The Subject Of The Contract. 9. The Contractor Is Obliged To Complete The Order On The Principles And Conditions Described In The Provisions Of The Contract Constituting Annex 3 To Swz.
