3.1. The Subject Of The Order Is The Supply Of Electronic Preposed Cards (Also Known As Vouchers) For Bearer For Employees Of The Board Of Municipal Cemeteries In Krakow, Divided Into 2 Parts, With The Following Division: 3.1.1. Part 1 Prepaid Cards (Later Swz Referred To As: Holiday) - For 153 Employees With A Total Purchasing Value Of Pln 180,200.00, With The Following Division: 3.1.1.1. 91 Prepent Cards With A Total Power Supply Value Pln 1,100.00 For 91 Employees. The Employer Allows The Delivery Of Two Pre -Paid Cards For A Given Employee With A Value, Which In Total Does Not Exceed Pln 1,100.00. 3.1.1.2. 22 Pcs. Prepaid Cards With A Total Power Supply Value Pln 1,200.00 For 22 Employees. The Employer Allows The Delivery Of Two Pre -Paid Cards For A Given Employee With A Value, Which Together Does Not Exceed Pln 1,200.00 Pln 3.1.3. 23 Pcs. Prepaid Cards With A Total Power Supply Value Pln 1,300.00 For 23 Employees. The Employer Allows The Delivery Of Two Preposed Cards For A Given Employee With A Value, Which In Total Does Not Exceed Pln 1,300.00. 3.1.1.4. 17 Pcs. Prepaid Cards With A Total Power Supply Value Pln 1,400.00 For 17 Employees. The Employer Allows The Delivery Of Two Preparing Cards For A Given Employee With A Value, Which In Total Does Not Exceed Pln 1,400.00. 3.1.3. The Validity Period Of Vouchers In The Form Of Prepayed Cards May Not Be Shorter Than Until 31/12/2026. In The Event Of When The Contractor Presents The Validity Period Shorter Than Until 31/12/2026, His Offer Will Be Rejected Due To The Content That Does Not Provide Swz Content. In Point 15.6. The Swz Has Been Determined By The Criterion According To Which The Contractor Has The Right To Extend The Validity Of Vouchers In The Form Of Pre -Paid Cards. This Point Also Includes The Procedure For Granting Points 3.1.4. The Contracting Authority Requires That The Contractor And The Offer Attach For Part 1 Or / Or For Part 2 List Of Sales Points Where Vouchers Can Be Realized In The City Of Krakow. The List Of Sales Points Is Not Supplemented Pursuant To Art. 128 Para. 1 Of The Act. 3.2. A Detailed Description Of The Subject Of The Contract Is Included In: 3.2.1. Planned Provisions Of The Contract For Parts 1, 2 (Annex 3 To Swz), 3.2.2. Description Of The Subject Of The Contract - For Parts 1 And 2 (Annex 4 To Swz).
