Tenders are invited for Supplying Medical Supplies to Al Mithaq Hospital - Rima Governorate Guarantee amount 500 $ Governorate Reema Note : Bids are submitted in US dollars. Required documents: - 1. A valid commercial registration. 2. A valid tax card. 3. A valid zakat card. 4. A bank guarantee (a payable check or a letter of guarantee valid for 90 days in the amount of $500). 5. A code of conduct and ethics signed and stamped by the supplier. 6. A supplier information form and a supplier verification and inspection form . 7. A valid professional practice license from the Supreme Authority for Medicines and Medical Appliances. 8. Technical specifications tables in English, signed and stamped on the back of each page. A file containing clear images of each item, accurately and comprehensively showing the four aspects of the product, must be attached to ensure proper technical evaluation. 9. A suppliers acknowledgment form of payment terms and conditions, signed and stamped with the official company seal. 10. Copies of previous contracts in the same field, with priority given to contracts executed with local and international organizations. · Tender documents will be uploaded starting Thursday 07/08/2025. · Quotations from applicants and all documents will be received in a sealed envelope on Tuesday 09/09/2025 from 10:00 AM to 4:00 PM at the Islamic Relief office in Sanaa.
