1. Subject Of The Contract: Provision Of Transport Services For 2 Disabled Students Residing In The Grodzisk Mazowiecki Commune From October 1, 2025 To June 26, 2026.2. A Detailed Description Of The Subject Of The Contract: 2.1 The Subject Of The Contract Is To Organize Transport With One Means Of Transport On The Route - Educational Institution - Place Of Residence For 2 Disabled Students, Residing In The Commune Of Grodzisk Mazowiecki: - 1 Student Residing In Makówka, - 1 Student Residing In Wólka Grodziska, Who Will Fulfill The School Obligation: • At Primary School No. 302 In Warsaw At Ul. Koszykowa 79B • Youth Sociotherapy Center In Warsaw At Ul. Jan Brożek 26 2.2. Disabled Students Will Be Transported In Accordance With The Calendar Of The School Year, The Institution They Attend, From October 1, 2025 To June 26, 2026, The Shortest Route Of Residence - Educational Institution - Place Of Residence. 2.3. Students Will Be Picked Up From Their Place Of Residence, At A Time That Guarantees Delivery To The School Facility For A Specific Hour, Resulting From The School Plan. The Collection Of Students From The Facilities Will Take Place After The Classes. It Is Allowed To Change The Delivery Deadlines, Resulting, Among Others From Students Sick Leave, Shortening Lessons Or Closing The School Facility. The Contractor Will Be Notified By This Situation By The Students Parent. 2.4. The Vehicle Carrying Students Must Be Insured And Adapted To The Needs Of Disabled Students In Accordance With The Act Of June 20, 1997. - Road Traffic Law (I.E. Journal Of Laws Of 2024, Item 1251, As Amended). 2.5. In Addition To The Driver, In The Vehicle, The Guardian Prepared For This Work Should Go With Students, In Accordance With Art. 39 Of The Educational Law, Which Will Take Care Of Students During Transport, Provides Help In Moving Between The House/School And The Means Of Transport, Will Perform The Necessary Care And Care Activities, And, If Necessary, Provide First Pre-Medical Aid. This Person Must Have A Completed Pre -Medical First Aid Course. 2.6. The Contractor Is Obliged To Care For The Students Until They Are Put Under The Care Of An Authorized Employee Of The Educational Institution, To Which They Attend, And In The Case Of Transport To Home Until He Is Put Into The Care Of An Adult Guardian. 2.7. It Is Reserved Not To Combine The Transport Of The Above Students With The Transport Of Other Students From The Grodzisk Mazowiecki Commune And Other Municipalities. 2.8. Due To The Possible Change In The Number Of Disabled Students, The Car Should Have At Least 1 Reserve Place. 2.9. Before Starting The Service, The Contractor Will Determine The Optimal Distribution, Indicating The Route And The Time Of Receipt Of Students, Having To Take Into Account The Facility In Accordance With The Class Plan And The Shortest Possible Time Of The Students On The Go.
