Special-Purpose Road Passenger-Transport Services-Sanitary Transport Service With Patients With Basic P-Type Type Type Type Type Syndrome. 1. The Subject Of The Contract Is The Sanitary Transport Service Of Patients With Basic P-Type Type Type Type Tribunal. 3. Pursuant To Art. 95 Para. 1 Of The Public Procurement Law, The Contracting Authority Requires Employment By The Contractor Or Subcontractor Under An Employment Contract Within The Meaning Of The Act Of 26 June 1974 Labor Code Of Labor Code (Journal Of Laws Of 2019, Items 1040, 1043 And 1495) Continuously Throughout The Entire Period Of The Contract Of Persons/Persons Performing The Dispatchers Activities In The Implementation Of The Subject Of The Contract. In Connection With The Above, During The Implementation Of The Contract, The Employer Is Entitled To Perform Inspection Activities Towards The Contractor Regarding The Fulfillment Of The Employment Requirement By The Contractor Or Subcontractor Under The Employment Contract Of Persons Performing The Above -Mentioned Activities During The Implementation Of The Subject Of The Contract. 6. At The Latest Before The Conclusion Of The Contract, The Contractor Shall Transfer To The Employer A Declaration Of Compliance With The Requirement Referred To In Para. 5, I.E. That A Person/Persons Performing Activities Related To The Implementation Of The Subject Of The Contract Will Be Employed Under An Employment Contract Within The Meaning Of The Act Of 26 June 1974, Labor Code. 7. The Ordering Party At Each Stage Of The Contract Has The Right To Verify The Contractor/Subcontractors Statement (Regarding The Form Of Employment Of The Persons Referred To In Paragraph 5). To This End, He Is In Particular To Ask The Proper Labor Inspectorate To Carry Out An Inspection. 8. The Purchaser Does Not Reserve The Possibility Of Applying For The Order Only By The Contractors Referred To In Art. 94 Of The Pzp Act. 9. The Purchaser Does Not Set The Requirements Referred To In Art. 96 Of The Pzp Act. 10. The Contractor Is Obliged To Have Voluntary Liability Insurance With A Value Not Lower Than Pln 1,000,000.00. At The Latest Before The Conclusion Of The Contract, The Contractor Will Provide The Employer With Documents Confirming The Fulfillment Of This Requirement.
