Tenders Are Invited For The Provision Of Specialized Care Services For People With Mental Disorders At Their Place Of Residence - Recipients Of The Municipal Social Welfare Center In Radzyń Podlaski.

1. The Subject Of The Contract Is The Provision Of Specialized Care Services For People With Mental Disorders At Their Place Of Residence - Who Are The Charges Of The Municipal Social Welfare Center In Radzyń Podlaski. 2. The Scope Of The Order Includes Conducting Rehabilitation And Rehabilitation Classes, Divided Into 2 Parts. This Order Applies To: Part 1: Psychological Therapy - In An Estimated Dimension Of 126 Hours. 3. For One Hour Of Service, The Service Is Understood 60 Minutes Of Actual Performance For The Benefit. Until The Service Is Provided, The Contractors Access Or Access Is Not Included. 4. Services Will Be Provided On The Basis Of Individual Administrative Decisions Issued By Mops In Radzyń Podlaski. 5. It Is Estimated That In 2025 About 6 Children And 2 Adults Will Be Covered By Services. The Number Of Benefits And Service Hours May Change, Due To The Variability Of Needs Resulting From The State Of Health Of The Pupils. 6. The Subject Of The Contract Includes In Particular: - Providing Children And Adolescents With Mental Disorders Access To Rehabilitation And Rehabilitation Classes, In Particular In Cases Where They Are Not Able To Use The Classes Referred To In Art. 7 Of The Act Of 19 August 1994 On The Protection Of Mental Health (Journal Of Laws Of 2024, Item 917, As Amended);- Providing Adults With Physical Rehabilitation And Improving Disturbed Functions Of The Body- In The Scope Not Covered By The Act Of 27 August 2004 On Health Care Services Financed From Public Funds (Journal Of Laws Of 2024, Item 146, As D.). 7. The Contracting Authority Does Not Cover Any Travel Costs Or Travel Expenses Related To The Provision Of Services. The Time Of Performing The Service Includes Only The Actual Time Of Performing Care Activities - The Time Of Travel, Access Or Return Of The Contractor Is Not Included. 8. Services Must Be Provided By Persons With Qualifications Specified In The Regulation Of The Minister Of Social Policy Of 22 September 2005 On Specialist Care Services (Journal Of Laws Of 2024, Item 816, As Amended). 9. The Contractor Is Obliged To Keep Documentation Of Services, Including:- Monthly Work Card Of The Person Providing Services, Containing: Date, Number Of Hours, Scope Of Activities Performed And Signature Of The Recipient (Or His Guardian) Confirming The Performance Of The Services. 10. The Place Of Provision Of Services Is The Place Of Residence Of The Mentee Or His Guardians In The City Of Radzyń Podlaski. 11. In The Event That The Recipient Uses Several Forms Of Support, The Services Cannot Be Provided At The Same Time.
