The Subject Of The Contract Is The Provision Of Specialized Care Services, Referred To In The Act Of 12 March 2004 On Social Assistance (Journal Of Laws Of 2024, Item 1283, As Amended) And In Executive Provisions, Hereinafter Referred To As Services (Excluding Specialized Services For People With Mental Disorders), Implemented At The Place Of Residence Of The Pupils Of The Pupils Of The Social Services Center In Lisew. Specialized Care Services Are Services Adapted To The Special Needs Arising From The Type Of Disease Or Disability, Provided By People With Specialist Professional Preparation. 3. The Following Types Of Specialized Services Are Established: 1) Teaching And Developing The Skills Necessary For Independent Life, In Particular: A) Shaping The Ability To Satisfy Basic Life Needs And The Skills Of Social Functioning, Motivating For Activity, Treatment And Rehabilitation, Conducting Training Skills And Social Skills And Supporting, Also In The Form Of Assisting In Everyday Life Activities, B) Intervention And Help In Family Life, C) Help In Dealing With Official Matters, D) Support And Assistance In Obtaining Employment, E) Assistance In Managing Money. 2) Care - As Supporting The Treatment Process, Including: A) Help In Access To Health Services, B) Agreeing And Guarding The Dates Of Medical Visits, Diagnostic Examinations, C) Help In Buying Or Ordering Medicines In The Pharmacy, D) Drugs And Observing The Possible Side Effects Of Their Use, E) In Particularly Justified Cases, Change Of Dressings, Help In The Use Of Auxiliary Measures And Medical Materials, Orthopedic Subjects, As Well As In Maintaining Hygiene, F) Help In Reaching Healthcare Facilities, G) Help In Reaching Rehabilitation Facilities. 3) Physical Rehabilitation And Improvement Of Disturbed Functions In The Field Undered By The Provisions Of The Act Of 27 August 2004 On Healthcare Services Financed From Public Funds ((Journal Of Laws Of 2024, Item 146), Including: A) In Accordance With Medical Recommendations Or A Specialist In Physical Rehabilitation Or Physiotherapy, B) Cooperation With Specialists In The Field Of Psychological, Pedagogical And Educational And Therapeutic Support Multilateral Activation Of A Person Using Specialized Services. The Above -Mentioned Specialist Services Can Be Performed Only By Persons Who Have Qualifications To Practice: A Social Worker, Psychologist, Pedagogue, Speech Therapist, Occupational Therapist, Nurse, Assistant To The Disabled Person, Environmental Guardian, Specialist In The Field Of Medical Rehabilitation, Physiotherapist Or Other Profession Giving Knowledge And Skills That Allow You To Provide Specific Specialized Services. Persons Providing Services Referred To In Part Iii, Point 3 Item 1 Letter A) Must Have Training And Experience In The Field Of: 1) The Ability To Shape Motivation To The Behavior Accepted By The Environment; 2) Shaping The Habits Of Deliberate Activity; 3) Conducting Training Of Social Behavior. 6.
