The Subject Of The Order Is To Conduct A Specialist Diagnosis Of Multidirectional Children Along With Specialist/ Therapeutic Support For The Child After Diagnosis. The Purpose Is Implemented As Part Of The Project Total Supermothy! Co-Financed By The European Union, Under European Funds For Silesia 2021-2027 (European Social Fund+), Priority: Fesl.07.00-European Funds For Society, Operation Fesl.07.07-Family, Children And Youth And Deinstitutionalization Of Foster Care. Specialist/Therapeutic For A Child After A Specialist Diagnosis, Multidirectional Children1) The Subject Of The Contract Includes Providing Specialized Diagnostic And Therapeutic Support For Children Enabling The Assessment Of Their Functioning And Improving Their Motor Skills, Support And Stimulating Their Physical And Mental Development, As Well As Improving Their Social Competence By Specialist Diagnosis, Multiple Children (Type: Diagnosis Of Attachment And Trauma Disorders, Diagnosis Of Neurodevelopmental Disorders, Si Diagnosis, Speech Therapy Diagnosis, Tus Diagnosis, Adhd Diagnosis, Fasd Diagnosis) .2) Diagnostic Support Will Be Implemented For Each Child Individually By A Team Of Specialists Set For The Performance Of The Order. 3) On The Basis Of The Diagnosis Implemented, An Individual Therapeutic Support Package For Each Child, In Which One Or Several Forms Of Therapy May Contain (Depending On The Deficits And Difficulties Of The Child And Areas Requiring Support), Each In The Appropriate, Tailored Number Of Hours May Be Developed. The Contractor Will Discuss With The Childs Guardian The Results Of The Diagnosis And Present An Individual Therapeutic Work Plan Along With The Opinion Regarding The Deficits Found In The Child In Writing. 4) The Final Number Of Hours Of Therapy For A Given Child Is Approved By The Contracting Authority Each Time. 15 Children Covered By The Diagnosis In The Period From The Signing Of The Contract To 31.10.2025, Min. 20 Children Covered By Diagnosis In 2026.C) Min. 25 Children Covered By The Diagnosis In 2027).
