The Subject Of The Contract Is To Provide A Disabled Student With A Decision On The Need For Special Education, Inhabited In The Dąbrówka Commune During Transport To School In The 2025/2026 School Year From The Town: 1) Dąbrówka To The 1St Degree Industry School No. 51 In Warsaw, Ul. Namysłowska 10, 03-455 Warsaw. 2. The Delivery Of Disabled Students Is Understood As: Bringing Disabled Students From The Dąbrówka Commune - To Schools And Educational Institutions From The Place Of Residence And The Students Reference To Their Place Of Residence. 3. The Transported Disabled Children Are Picked Up And Transported To The Address Provided By The Guardian4. Vehicles Intended For Transport Must Correspond To The Provisions Of The Road Traffic Act. 5. License For The Performance Of National Road Transport Of Persons, A Certificate Of Competence In National Road Transport Of Persons Issued For One Of The People Managing The Contractor6. The Contracting Authority Requires That The Transport Takes Place With Technically Efficient Vehicles For Transporting Disabled Students. In Autumn And Winter Periods, Students Bringing Students Must Be Properly Heated. 7. The Offer Includes The Price Of The Offer For One Day Of Delivery Of One Child Participating In The Classes. The Price Must Take Into Account All Costs. 8. The Estimated Number Of Days In The 2025/2026 School Year Is 188 Days. Detailed Customs Related To The Subject Of The Contract: 1. The Contractor Applies To The Highest Diligence In The Performance Of The Subject Of The Contract. 2. The Contractor Is Responsible For The Overall, Including The Course And Timely Performance Of The Contract. 3. Vehicles Must Provide Seating For All Transported People, Vehicles Must Be Properly Marked - Transport Of Disabled People. 4. The Contracting Authority Requires From The Carrier That There Must Be A Guardian In Every Vehicle Providing A Delivery Service. The Driver Or Guardian Must Have A First Aid Course. 5. The Contracting Authority Does Not Allow Children To Be Transplanted During Delivery To Another Vehicle Outside Of Special Reasons. 6. The Duration Of The Delivery/Transport For A Individual Child Cannot Be Longer Than 1St. 7. Traveling Hours (Access And Return) From Monday To Friday, On School Days In Accordance With The Established Class Plan. 8. The Contractor Is Obliged To Ensure Care For Students During The Delivery And On The Way Back And Maintain Special Safety During The Transport Of Students9. The Contractor Will Ensure That The Guardian Brought Children From The Car To School And From School To Car. 10. The Collection Of Students Will Take Place From Their Place Of Residence (The Exact Addresses Will Be Indicated When Signing The Contract) 11. The Number Of Students Covered
