1. The Subject Of The Contract Is The Successive Delivery Of Milk And Dairy Products For The Needs Of The Nursing Home In Zawiercie In The Fourth Quarter Of 2025.2. A Detailed Type Of Range And Scope Of The Order Was Specified In The Assortment And Price Form, Constituting Annex 1.1 To Swz. The Quantities Of Individual Goods Given In This Form Are Amounts Expected By The Ordering Party, Which Were Accepted To Compare Offers And Select The Most Advantageous Offer. The Employer Reserves The Right To Change The Number Of Individual Goods Depending On The Current Needs While Maintaining Unit Prices And Up To The Amount Of The Future Public Procurement Contract And The Right To Limit The Contract Value By A Maximum Of 30 %.3. Goods Covered By This Order And The Method Of Their Delivery Must Comply With The Provisions Of The Specification Of The Terms Of The Contract, The Assortment And Price Form And The Standards In This Respect In This Respect, As Well As The Legal Provisions Appropriate For The Subject Of The Contract. 4. The Place Of Delivery Is The Seat Of The Ordering Party: Nursing Home In Zawiercie, Ul. Rzemieślnicza 9, 42-400 Zawiercie. 5. The Implementation Of The Order Must Comply With The Principles Of The Haccp.6 System Implemented And Used In The Department. The Contractor Will Provide The Goods With A Means Of Transport Adapted For Food Transport In Accordance With Applicable Regulations, Under Pain Of Not Accepting Delivery By The Contractor. 7. At Each Written Request Of The Ordering Party, The Contractor Will Allow The Control Of The Means Of Transport And Containers For The Transport Of Goods In Terms Of Their Purity. 8. The Contractor Will Present (If Concerned) To Each Written Request Within The Time Limit Indicated By The Contracting Authority: 1/ Decision Of The State Poviat Sanitary Inspector For The Means Of Transport, Which Allows The Transport Of Milk And Dairy Products, 2/ Current Medical Certificate From The Examination For Sanitary And Epidemiological Purposes Of The Person Supplying The Goods. 9. Deliveries Will Be Carried Out Successively, Depending On The Current Needs Of The Ordering Party, Based On Separate Orders, Determining Min. Quantity, Product Range And Delivery Date. 10. The Contracting Authority Requires That Deliveries Take Place On Business Days, Five Times A Week. 11. The Contractor Will Provide Delivery From 6:30-8:00 On The Date Indicated In The Electronic Message By The Contracting Authority. This Term May Not Be Less Than 2 Business Days, And The Day Of Delivery Cannot Be A Public Holiday. 12. Other Requirements Of The Contractors And The Manner And Detailed Conditions For The Implementation Of The Subject Of The Contract, As Well As The Payment Terms, Specify The Proposed Provisions Of The Future Contract Constituting Annex 2 To Swz.
