Name Of Bid : Implementation Of Nat Testing Facility With Id Methodology Having Available Technologies Covering Blood Centres Of The State Of Odisha On Ppp Mode Bid Reference No.: Osmcl/2025-26/Id Nat Services Ppp Retender/09 Bidder Name : Price Schedule 1. This Boq Template Must Not Be Modified/Replaced By The Bidder And The Same Should Be Uploaded After Filling The Relevent Columns, Else The Bidder Is Liable To Be Rejected For This Tender. Bidders Are Allowed To Enter The Bidder Name And Values Only. Multiple Price Bid Submission By Bidder Shall Lead To Cancellation Of Bid. 2. A Single Rate For All Types Of Test Under Id Nat Screening Per Valid Donation Along With All Incidental Charges Has To Be Quoted In The Price Bid Boq (Column 3 Of The Excel File) Excluding The Gst Which Shall Remain Fixed For The Entire Duration Of Contract And Will Not Be Subjected To Any Escalation. It Is To Note That No Additional Cost Except The Quoted Rate Per Valid Donation Shall Be Claimed During The Contract Period. 3. The Price (Cost For All Types Of Test Under Id Nat Screening Per Valid Donation) To Be Quoted Should Include All Factors Like Supply & Installation Of Equipment, Proper Earthing, Supportive Infrastructure, Hr, Electricity Bill Of The Id Nat Testing Facility (Through Separate Meter/Sub Meter) , All Consumables, Reagents, Controls, Calibrators Required To Run The Test, Operational, Maintenance (Comprehensive Maintenance Contract (Cmc) With The Oem) And Periodic Calibration Cost, Cost Towards Sample Collection & Transportation , Security Personnel, Housekeeping/Cleaning Staffs Or Any Other Expenses Incurred In The Day To Day Running Of The Id Nat Testing Facility Along With All Incidental Charges For The Project, As Mentioned In The Scope Of Work And Terms & Condition. 4.
