The Subject Of The Order Is The Delivery And Implementation Of An It System/Software For The Department Of Pathology I And Ii And The Central Molecular Laboratory, Histocompatibility Antigen Testing Laboratory (Hla) At The Lower Silesian Center For Oncology, Pulmonology And Hematology, Together With Installation, Parameterization And Configuration, As Well As Training In The Use Of The System, As Well As Service Services And Authors Supervision. 3.2. Detailed Description Of The Subject Of The Order Referred To In Point 3.1., Is Included In Annex No. 1 To The Swz - Description Of The Subject Of The Order (Opz), Annex No. 3 - Assortment And Price Sheet, Annex No. 7 To The Swz - Draft Contract. 6.1. In Order To Initially Confirm The Lack Of Grounds For Exclusion From The Proceedings And The Fulfillment Of The Conditions For Participation In The Proceedings, Pursuant To Art. 125 Section 1 And 2 Of The Public Procurement Law, The Contractor Submits, Together With The Offer With A Qualified Electronic Signature, The Following Documents: 1. Completed And Signed By Persons Authorized To Represent The Contractor: Opz - Annex No. 1 To The Swz, Assortment And Price Sheet - Annex No. 3 To The Swz, Offer Form - Annex No. 2 To The Swz. 2. The Declaration In The Form Of The Single Procurement Document (Hereinafter Referred To As The Single European Procurement Document (Hereinafter Referred To As The Single European Procurement Document) (Hereinafter Referred To As The Single European Procurement Document (Hereinafter Referred To As The Single European Procurement Document) As At The Date Of Submission Of Offers) Shall Be Prepared, Under Pain Of Nullity, In Electronic Form And Shall Be Signed With A Qualified Electronic Signature – Annex No. 4 To The Swz). 3.Declarations Of The Contractor/Contractor Jointly Applying For The Contract Regarding The Grounds For Exclusion Under Art. 5K Of Regulation 833/2014 And Art. 7 Section 1 Of The Act On Special Solutions For Counteracting Support For Aggression Against Ukraine And For The Protection Of National Security - A Template Of The Declaration Is Attached As Annex No. 4A To The Swz. 4.Declarations Of The Entity Providing Resources Regarding The Grounds For Exclusion Under Art. 5K Of Regulation 833/2014 And Art. 7 Section 1 Of The Act On Special Solutions For Counteracting Support For Aggression Against Ukraine And For The Protection Of National Security - Annex No. 4B To The Swz. 5. If The Contractor Relies On The Capabilities Or Situation Of Entities Providing Resources, Together With The Declaration Referred To In Point. 2, Also Declarations Of The Entity Providing The Resources (Appendix No. 4B To The Swz), Confirming That There Are No Grounds For Excluding This Entity And That The Conditions For Participation In The Proceedings Are Met. A Template Of The Obligation Of The Entity Providing Resources To Provide The Contractor With The Necessary Resources For The Purpose Of Implementing The Order (If Applicable) Is Attached As Annex No. 8 To The Swz. 6. In The Case Of Contractors Jointly Applying For The Award Of A Contract, The Declarations Referred To In Point 2 And Point 3 Is Submitted By Each Contractor. These Declarations Confirm The Lack Of Grounds For Exclusion And The Fulfillment Of The Participation Conditions To The Extent That Each Performer
