The Subject Of The Order Is The Delivery And Implementation Of An It System/Software For The Department Of Pathology I And Ii And The Central Molecular Laboratory, Histocompatibility Antigen Testing Laboratory (Hla) At The Lower Silesian Center For Oncology, Pulmonology And Hematology, Together With Installation, Parameterization And Configuration, As Well As Training In The Use Of The System As Well As Service Services And Authors Supervision In As Part Of The Project Titled: Purchase Of Equipment In The Oncology-Pulmonology-Hematology Hospital In Wrocław In The Field Of Aos And One-Day Services - Stage I No. Feds.05.01-Iz.00-0003/24, Financed By The European Funds For Lower Silesia 2021-2027 Program. 3.2. Detailed Description Of The Subject Of The Order Referred To In Point 3.1., Can Be Found In Annex 1 To Swz - Description Of The Subject Of The Order (Opz), Annex No. 3 - Assortment And Price Sheet, Annex No. 7 To Swz - Draft Contract. 6.1. In Order To Initially Confirm The Lack Of Grounds For Exclusion From The Proceedings And The Fulfillment Of The Conditions For Participation In The Proceedings, Pursuant To Art. 125 Section 1 And 2 Of The Public Procurement Law, The Contractor Submits, Together With The Offer With A Qualified Electronic Signature, The Following Documents: 1. Completed And Signed By Persons Authorized To Represent The Contractor: Opz - Annex No. 1 To The Swz, Product And Price Sheet - Annex No. 3 To The Swz, Offer Form - Annex No. 2 To The Swz. 2. A Declaration In The Form Of A Single Procurement Document (Hereinafter Referred To As: Jedz) And A Declaration In The Form Of A Single European Procurement Document (Hereinafter Referred To As Jedz/Single Document), Shall Be Prepared, Under Pain Of Nullity, In Electronic Form And Shall Be Provided With A Qualified Electronic Signature - Annex No. 4 To The Swz. 3.Declarations Of The Contractor/Contractor Jointly Applying For The Contract Regarding The Grounds For Exclusion Under Art. 5K Of Regulation 833/2014 And Art. 7 Section 1 Of The Act On Special Solutions For Counteracting Support For Aggression Ukraine And Serving To Protect National Security - The Template Of The Declaration Is Annex No. 4A To The Swz. 4.Declarations Of The Entity Providing Resources Regarding The Grounds For Exclusion Under Art. 5K Of Regulation 833/2014 And Art. 7 Section 1 Of The Act On Special Solutions For Counteracting Support For Aggression Against Ukraine And For The Protection Of National Security - Annex No. 4B To The Swz. 5.Contractor, In Case Of Reliance Capabilities Or Situation Of Entities Providing Resources, Is Presented Together With The Declaration Referred To In Point 2, Also Declarations Of The Entity Providing The Resources (Appendix No. 4B To The Swz), Confirming That There Are No Grounds For Excluding This Entity And That The Conditions For Participation In The Proceedings Are Met.
